Construction of the Leonardville Nursing Home

  Our Beginnings

Our story began in 1966, when several area churches recognized a need for quality long-term care within the community. Representatives from each church came together to form an advisory board, laying the foundation for what would become our home. From there, a smaller board of directors was elected to guide our mission. On August 1, 1967, our doors officially opened, marking the start of a tradition built on faith, compassion, and community care.

Growth Through the Years

Over the decades, our home has continued to expand and evolve to better serve our residents:

Our Culture
of Care

In 2012, we began our transformative journey in Culture Change. Through education and partnership with long-term care experts, we embraced the Person-Centered Care model promoted by the Promoting Excellent Alternatives in Kansas Nursing Homes (PEAK) program. This philosophy reshaped our approach—putting the individual needs, choices, and dignity of each elder at the heart of everything we do.

Neighborhood Living

On July 1, 2013, we introduced three unique neighborhoods: Serenity House, Harmony House, and Kindred House.

Each neighborhood is home to 12–25 elders and features its own kitchen, cozy living area, and spa. This small-scale, family-style setting fosters meaningful relationships, independence, and a true sense of home.

Awards and Recognition

Our commitment to excellence has been recognized across Kansas:

  • 2016: Person-Centered Care Home Award

  • 2017: Celebrated 50 years of service

  • 2018-2025: PEAK Mentor Home Awards

We are proud to be one of only nine PEAK Mentor Homes in Kansas, reflecting our dedication to innovation, compassion, and quality in elder care.

What Our Staff Say About PEAK

"The Peak recognition is an honor to us all as it reminds us of the goals that we set years ago to make long term care feel like a relationship experience instead of a loss of identity for people leaving their personal living situations. It is a reminder that people deserve to be treated with dignity and that does not change with age. With Peak we have grew as a team and as a family entity. When you make changes that grow bonds, support each person's choice, and give everyone a voice...you have given them their purpose and that in itself feeds the soul."

Our journey continues as we seek new and creative ways to bring joy and connection into daily life. Through collaboration, innovation, and heartfelt care, we remain committed to creating a community where every elder feels valued, respected, and truly at home.
